Special Needs Childcare Costs by State (2026)

ABA therapy runs $2,600–$5,400/month depending on your state. That's $31,200–$64,800/year before insurance. The good news: all 50 states mandate commercial insurers cover ABA. The bad news: copays, age caps, and session limits make the real out-of-pocket wildly different state to state.

Insurance Changes Everything

Without insurance, ABA therapy costs $40,000-$65,000/year. With insurance (which every state now mandates for autism), most families pay $50-$200/month in copays. The catch: many plans cap at 25-30 hours/week, have age limits (often up to age 19-21), or require prior authorization every 6 months. Check your specific plan. Medicaid covers ABA with no copay in most states.

Types of Special Needs Childcare

ABA therapy (Applied Behavior Analysis)

The most common autism treatment. Sessions run 25-40 hours/week for young children, delivered by a BCBA (Board Certified Behavior Analyst) and RBTs (Registered Behavior Technicians). Costs $100-$200/hour without insurance. Most effective when started before age 5.

Therapeutic daycare

Full-time daycare with integrated therapy services: speech, OT, behavioral support. Staff ratios are 1:3 or 1:4, compared to 1:8 or 1:10 at standard centers. Costs roughly 60-80% more than regular daycare in the same area. Some accept CCDF subsidies.

Early intervention (Part C)

Federally mandated services for children birth to age 3 with developmental delays. Free in most states regardless of income. Covers speech therapy, occupational therapy, developmental support, and family training. Referrals through your pediatrician or state early intervention program.

Inclusive preschool programs

Ages 3-5 with IEPs receive free services through the school district (Part B of IDEA). The district must provide a "free appropriate public education" in the least restrictive environment. This can include specialized preschool classrooms with speech, OT, and behavioral supports at no cost.
